Gathering patient data from the home provides the evidence needed to make the best possible clinical decisions. This data can empower caregivers, nurses and patients. Healthcom provides wireless in-home patient monitoring systems that let you build a 'digital dashboard' for each patient. You decide, on a per-patient basis, exactly what to measure; we provide the toll-free communcations network and secure data repository.

These important values are captured and displayed on a secure website where they can be checked at any time. Values over your defined limits can cause an immediate alert (pager, fax, phone call, e-mail, etc.). Your clinicians can know immediately when a patient is out of protocol and can choose the proper intervention ... phone call, visit or contacting a family member.

We use a series of standard devices manufactured by well-known device makers, FDA and UL approved, and available without proprietary contracts. All are Bluetooth-enabled to work with our Gateway, and comply with memory, protocol and data requirements that enable Healthcom to post measurements to CareNet for caregiver review.

Using standard appliances dramatically reduces both start-up and replacement cost., while allowing Healthcom, and you, to take full advantage of new devices and technologies that emerge.

Care Net
CareNetData from Healthcom's telehealth devices (and other services) are conveniently posted on CareNet where they can be reviewed within the context of other systems and devices deployed with your client. For example, weight and blood pressure could be evaluated with knowledge of medication compliance or non-compliance.

Falls and ambulance dispatches can be contextually related to medicine, blood pressure, weight, etc. There is simply no better or easier system available. It is patient-, family- and nurse-friendly.

For telehealth devices, data can be selected and graphed as a trending curve over time, yielding trajectory information such as rise in systolic pressure over a 1 month period. Your nurse can set alert parameters separately for each patient.

Healthcom's FYI service can keep your clinical team up to date with automatic email and text message alerts which can be setup to follow customized alert notification rules.

There are no seat licenses, software maintenance fees or other hidden fees. Access to the web -based monitoring platform is included in the low monthly monitoring fees.
